Dear Patients,

We are striving to ensure that we are taking every precaution to protect the health and safety of our team and our patients so that we can continue to offer care for patients who need it

At this time, Revive will continue to offer limited hours and appointments for emergent care for our existing patients. 

Emergent care is defined as follows:

  1. A patient is in an acute condition where they would be severely physically limited from basic daily activities without care.
  2. A patient is in a sub-acute condition that if left untreated would be vulnerable to an acute flare-up that could leave them severely physically limited from basic daily activities.

Currently, we are not accepting new patients. 

If you are an existing patient seeking emergent care, please call the clinic at 780-460-7384 or email: frontdesk@revivehealth.ca   We will return your message as soon as possible.   

If you are not seeking emergent care, please check back with us on April 1, 2020 for an update.   

If you would like to speak to your practitioner or are not seeking emergent care but still need some guidance – we are here for you!   

We are available to provide guidance and direction on your care plan.  Please call the clinic and you will be connected with the clinician you are looking for.

Upon booking an appointment you will receive a pre-screening questionnaire via email or text.  We ask that you answer it truthfully and send it back signed prior to entering the clinic. 

Even if you qualify for an emergent care appointment,  please do not book an appointment or enter the clinic if any of the following apply to you: 

  • If you have travelled anywhere within the last 14 days
  • Are caring for someone in isolation
  • Suspect you have been exposed to COVID -19 or are showing signs of the virus including:
  • fever, sore throat, SOB, nausea, or generally feel unwell in any way. 
  • Are not observing AHS’s recommendations on social distancing. 

If you have an emergent care appointment:

Our waiting room is closed.   This means you will either be asked to wait in your car until the time of your appointment and will be called when your practitioner is ready to see you. 

Upon entering the clinic you will be asked to wash your hands.  

You will be taken directly to a treatment room. 

You will be asked to pre-pay for your appointment so to limit the time spent in the clinic and possible exposure to our admin staff.  Your receipt will be emailed to you.
